    2. Ionizing radiation sources
      1. Requirements for human use (NRC #: State of North Carolina License Number N.C. 074-296-3 may be used in application when requested.)
        1. Radioactive materials, e.g., Tc- 99m scan--Requires hospital privileges and an approved application from the Radiation Safety Committee
        2. Sealed sources, e.g., Ir-192 brachytherapy--Requires hospital privileges and an approved application from the Radiation Safety Committee
        3. Diagnostic x-rays, e.g., fluoroscopic examinations--Requires hospital privileges and registration by the Radiation Safety Committee
        4. Accelerators, e.g., cancer management--Requires hospital privileges and an approved application from the Radiation Safety Committee
      2. Requirements for nonhuman use (NRC #: State of North Carolina License Number N.C. 074-296-1 may be used in application when requested.)
        1. Radioactive materials, e.g., P- 32 labeling--Requires an approved application from the Radiation Safety Committee
        2. Sealed sources, e.g., Cs-137 cell irradiation--Requires registration with the Office of Radiation and Biological Safety
        3. X-rays, e.g., x-ray fluorescence--Requires registration with the Office of Radiation and Biological Safety
    3. Nonionizing radiation sources
      1. Requirements for human use
        1. Microwaves, e.g., hypothermia treatment--Requires hospital privileges
        2. Lasers, e.g., laser surgery-- Requires hospital privileges
        3. Radio frequency units, e.g., hypothermia treatment--Requires hospital privileges
      2. Requirements for nonhuman use
        1. Microwaves, e.g., heating--No registration or application required
        2. Lasers, e.g., physics experiments--Class III and greater require registration with the Office of Radiation and Biological Safety
        3. Radio frequency units, e.g., heating devices--No registration or application required

  2. Biological Hazards (Refer to ECU Biological SafetyManual)
    1. Descriptions of materials and guidelines for use available from:
      1. ECU Biological Safety Manual
      2. Office of Radiation and Biological Safety. See the contact information sheet included in this packet.
    2. Requirements for infectious microorganisms
      1. Registration for work with any Class 2 agents (See Biological Safety Manual for definitions.)
      2. Class 3, 4, or 5; human hepatitis (all agents); human retroviruses; and nonhuman primate retroviruses-- Approved application by the Biological Safety Committee
    3. Requirement for recombinant DNA molecules
      1. Exempt molecules do not require registration (See Biological Safety Manual for definitions.)
      2. Nonexempt molecules require an application and approval of the Biological Safety Committee
    4. Requirements for human tissues and body fluids
      1. Registration with the Office of Radiation and BiologicalSafety
    5. Requirements for experimental animals
      1. Application and approval required for:
        1. 1) Transplantation or injection of human tissues into animals
        2. Use of nonhuman primates
        3. Use of nonhuman primate tissues or use of human or nonhuman primate retroviruses

  3. Hazardous Chemicals (Refer to ECU Chemical Hygiene Manual)
    1. Descriptions of materials and guidelines for use available from:
      1. ECU Chemical Hygiene Manual
      2. Department of Environmental Health and Safety. See the contact information sheet included in this packet.
    2. Material required
      1. Possession and retention of Material Safety Data Sheets
      2. Specific requirements exist for certain chemicals. Consult with the University Policy and Procedure for the Use of Hazardous Chemicals in the ECU Chemical Hygiene Manual
